American Journal of Respiratory Critical Care Medicine; 1999 Sleep-disordered breathing (SDB) is recognized as a serious health problem that impacts the cardiovascular system. Clinical studies have shown there is a high prevalence of SDB in heart failure patients. Furthermore, recent data suggests that these comorbid patients suffer equally from two distinct types of SDB, obstructive sleep apnea (OSA) and central sleep apnea (CSA). Heart failure patients with CSA typically exhibit a form of periodic breathing, known as CheyneStokes respiration (CSR), and these patients have been shown to have a very poor prognosis. Currently available positive airway pressure (PAP) therapies have had mixed results for patients with CSA/CSR. To this end, ResMed has developed the VPAP Adapt SV, the first FDA cleared PAP device to treat CSA/CSR in a home environment. This novel therapy utilizes adaptive servo-ventilation (ASV) and has demonstrated the ability to improve clinical outcomes and quality of life in heart failure patients. Cheyne-Stokes respiration in patients with congestive heart failure: causes and consequences.

Cheyne-Stokes respiration is a form of periodic breathing in which central apneas and hypopneas alternate with periods of hyperventilation, producing a waxing and waning pattern of tidal volume. This review focuses on the causes and consequences of Cheyne-Stokes respiration in patients with congestive heart failure, in whom the prevalence is strikingly high and ranges from 30% to 50%. Several f...

Effect of flow-triggered adaptive servo-ventilation compared with continuous positive airway pressure in patients with chronic heart failure with coexisting obstructive sleep apnea and Cheyne-Stokes respiration.

BACKGROUND In patients with chronic heart failure (CHF), the presence of sleep-disordered breathing, including either obstructive sleep apnea or Cheyne-Stokes respiration-central sleep apnea, is associated with a poor prognosis. A large-scale clinical trial showed that continuous positive airway pressure (CPAP) did not improve the prognosis of such patients with CHF, probably because of insuffi...

Effect of oxygen on sleep quality, cognitive function and sympathetic activity in patients with chronic heart failure and Cheyne-Stokes respiration.

BACKGROUND Cheyne-Stokes respiration disrupts sleep, leading to daytime somnolence and cognitive impairment. It is also an independent marker of increased mortality in heart failure. This study evaluated the effectiveness of oxygen therapy for Cheyne-Stokes respiration in heart failure. METHODS Eleven patients with stable heart failure and Cheyne-Stokes breathing were studies. Oxygen and air ...

Pathophysiology and treatment of Cheyne-Stokes respiration.

Cheyne-Stokes respiration is a disorder characterised by recurrent central apnoeas during sleep alternating with a crescendo-decrescendo pattern of tidal volume. 2 It is often observed in patients with congestive heart failure, usually during stages 1 and 2 non-REM sleep when ventilation is under chemical-metabolic control. Patients with Cheyne-Stokes respiration usually present with the sympto...

Reversal of central sleep apnea with oxygen.

OBJECTIVE To examine the effect of oxygen on apneas and sleep quality in patients with frequent central apneas during sleep. DESIGN/SUBJECTS Prospective intervention study of 20 consecutive patients with predominant central apnea identified from 570 patients referred for suspected sleep apnea syndrome. Sixteen patients had congestive heart failure and seven of them had a previous stroke. Thre...

Obstructive and Central Sleep Apnea and the Risk of Incident Atrial Fibrillation in a Community Cohort of Men and Women

BACKGROUND Previous studies have documented a high prevalence of atrial fibrillation (AF) in individuals with obstructive sleep apnea (OSA). Central sleep apnea (CSA) has been associated with AF in patients with heart failure. However, data from prospective cohorts are sparse and few studies have distinguished the associations of obstructive sleep apnea from CSA with AF in population studies. ...
